Historical Archive - This paper inquiry questionnaire may or may not still be in use by the Provincial School Attendance Counselor's appointed inquiry officers. The OFTP considers the questions to betray an institutional bias concerning pedagogical methods. We therefore recommend insisting on an in-person hearing if you should ever be subject to a provincial inquiry. You may request the participation of an OFTP representative.
